Who can refer a child? 
The following people with detailed knowledge of the oncology patient’s current medical condition or in the event of demise, knowledge of diagnosis and death. 

  • Potential “Spectacular Siblings” who meet the eligibility criteria.

  • Parents and legal guardians.

  • Medical Professionals
    (a physician, nurse, social worker or child-life specialist).

  • Family members, pastors and friends. 

​Eligibility criteria:
A child who has reached the age of 4 years old and is younger than 18 years old at the time of referral. In addition, the child must have a sister or brother that meets one of the following criteria:


  1. A current oncology patient being treated for cancer at the time of referral.

  2. Oncology patient within five years of referral date.

  3. Have a sibling who died from cancer within five years of referral completion. 

Submission of a medical referral form completed by the cancer patient’s oncologist, primary care provider, or social worker is required. This form is acceptable for deceased siblings or you may submit proof of demise (ie; obituary or certificate of death). Please complete the Referral Form to begin the admission process. 

Confidentiality Policy

The Forever Jaiden Foundation, Inc. will not disclose any personal information about our families or their children (i.e. medical, demographic, contact information, etc.), to any persons or organizations without prior written consent, other than governing agencies as required by law.
